it's actually 100% operators fault. I work with 9 of these robots by myself, 6 days a week. there are safety precautions we are trained to take. the main computer controlling them, set to Maintenance Mode, this homes out all robots in order to be worked on. each robot you have to interact with, gets set to manual mode by key @ said robots controlbox, or it gets LOTO (lock out tag out, and powered down) depanding on what you're doing. we also keep the doors opened, leading inside the caged area because its an additional "E-Stop", nothing runs, unless door is closed, main control unit is set to auto, and the robots themselves set back to auto by a physical key. we use ABB IRB 6600's mainly
