There's so many components working together to make an overridable self driving system possible, and failure to any of those components can cause a crash.
Yes humans are part of the system, and it could be a human error, but any sensor or actuator failing can also cause cascading failure.
I think it's a bit biased that out of all these factors, it's the man you singled out as the culprit.
