You might ask your doctor to provide a letter about your situation, diagnosis, and treatment. Something you can scan and send, or an email with contact info, to provide along with your communications with the new employer.
If you explain the whole new job thing to a nurse, or the doctor themselves, I'd like to think they would take the time to help you at least explain the situation so the new employer can feel like you are being honest and up front, and that this is out of your control.
